The person completing this form must be of legal age or the legal parent or guardian of the minor participant and must fully understand the contents and implications thereof.
ONLINE CLASS POLICY & WAIVER: You agree that by participating in online physical exercise or online training activities, you do so entirely at your own risk. You agree that you are voluntarily participating in these activities and using your facilities and premises and assume all risks of injury, illness, or death. By participating in our online classes, you expressly agree to release and discharge the trainer or instructor from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against the trainer or instructor for personal injury or property damage.
MEDIA RELEASE: By signing this form, I grant my permission to Sharpen Up! Dance Team Training, and its owning entities, to use my child's photograph publicly to promote their business. I understand these images may be used in print publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to be by reason of such use.
