LIABILITY RELEASE: We, the staff of Sharpen Up! Dance Team Training, recognize our obligation to make sure our students and their parents are aware of the risks and hazards involved in the sport of dance. By signing this release form, you release Sharpen Up! Dance Team Training and all the employees from all claims on account of any injury or illness which may be sustained by your child attending any class or outside performances associated with Sharpen Up! Dance Team Training. You also affirm you now have, and will continue to carry, proper primary medical, health, hospitalization, and accident insurance, which you consider adequate for protection of both your child and Sharpen Up! Dance Team Training. MEDIA RELEASE: By signing this form, I grant my permission to Sharpen Up! Dance Team Training, and its owning entities (Danielle's Pom Training, Inspiration Dance Choreography, and Radiance Dance Training & Fitness), to use my child's image publicly to promote their business. I understand these images may be used in print publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to be by reason of such use.
